Requirements:

Board Eligible (within 5 years of residency) or Board Certified in Urology - [REQUIRED]
Active CO License (or willingness to obtain) - (note in "Additional Submission Details" field)- [REQUIRED]
BLS and ACLS - [REQUIRED]
Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation

DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - [REQUIRED] * no exemptions
DEA Controlled Substance License (required once hired)
